How to Report a Supplement Side Effect: 911 First, Then FDA and the Seller
If you feel seriously unwell after a supplement, stop it and get help first; call 911 for sudden weakness, confusion, trouble speaking or trouble breathing. Afterward, report to your clinician, to FDA through its MedWatch program and to the seller. Keep the bottle, the panel photo and the lot number.

First things first01
What should you do first if you feel unwell?
Stop taking the supplement, get care, and call 911 right away for sudden weakness, confusion, trouble speaking or seeing, or a sudden severe headache.
- Sudden confusionConfusion, trouble speaking or understanding speech that starts suddenly.
- Face, arm or legNumbness or weakness, often on one side of the body.
- Vision and balanceTrouble seeing, dizziness, loss of balance or poor coordination.
- Sudden severe headacheA severe headache with no known cause.
- Call 911, do not driveAny of these: call 911 right away and note when it started. Do not drive yourself or the person to the hospital.
Stroke signs from the CDC 1. This list is not complete, and a supplement is never a reason to wait.
Reporting comes after safety. A form can wait an hour, a hospital visit cannot. Do not drive yourself.

What details make a report useful?
Dates, doses, lot number and a photo of the panel make a report useful, because they let someone match your problem to a product.
| Detail | Where to find it |
|---|---|
| Product and panel | Photo of the Supplement Facts box |
| Lot number | Base or side of the bottle |
| Dates and capsules a day | Your own notes and the order email |
| Other medicine | Pharmacy labels |
| What happened, and when | A short dated note, written soon |
| What care you got | Visit summary or discharge sheet |
Do not throw the bottle away, even if it is empty. Our batch page shows where lot numbers sit.
Who to tell03
Who should you report to, and in what order?
Tell your clinician first, then report to FDA, then the seller.
Your clinician or pharmacist
They can act on it and add it to your record.
FDA
FDA runs a program called MedWatch for reports of problems with supplements. Search for it on fda.gov and file online. NCCIH also asks consumers to report safety concerns 2.

What can a short report look like?
Keep it factual: what you took, when, what happened and what you did about it.
Product and lot. Panel on the bottle. Start date and capsules a day. Other medicine. Symptoms and the date they began. Whether you stopped. Care you received. Contact details.
Leave out guesses about cause. You are reporting what happened, and others will judge the link. Keep a copy of every message and the date you sent it.

What about refunds, and what happens to the bottle?
Health comes first and the refund can wait, so ask the order desk how to return bottles only after you have the care you need.
The refund policy describes the steps, and the sales page describes a 60-day guarantee. Keep the empty bottle until the desk says what to do. If you decide to order again later, read your panel and talk to your pharmacist first.

FAQ06
HoneyPezil questions people ask
Do I need proof the supplement caused it?
No. A report describes what happened. You do not have to prove cause, and you should not guess at one.
Can the website inbox take my report?
No. The website inbox cannot see orders or open claims. Use the order desk for the seller report.
What if I am not sure it was the supplement?
Report it anyway and say you are unsure. Note what else changed that week, such as new medicine or illness.
Should I report a mild upset stomach?
You can. Tell your pharmacist and note the lot. A mild effect that lasts or worsens is worth a call.
